Age policies
Items in a state for too long get flagged. Stale stories get triaged before they rot.
Definition of Ready / Done
Required fields per state. Acceptance criteria, story points, owner — never optional again.

Why we're building it.
Every engagement we've run has hit the same backlog problem: agreed-on policies that quietly stop being followed by sprint 4. Telling the team to "be more disciplined" never works.
